Who This Form Is For:
This form is for foster and kinship placements only.

Foster Families: The Welcome Pack program is designed to support new placements and should be requested within the first 30 days of placement.
Kinship Families: Support can be requested within the first 6 months of placement.

Reunifying Families & Aged out Youth:
If you are supporting a reunifying family or you are a parent reunifying with your children, please note that this is not the correct form. We have a separate form for reunification support, which can be completed by a parent, caseworker, foster parent, or CASA when needed. Email aker@fostervillagedfw.org to get that form!

Project SOOTHe (Sensory Opportunities Offering Therapeutic Help) provides sensory-based support designed to help children regulate, feel safe, and thrive through individualized sensory opportunities. Kara is our Project SOOTHe Coordinator, and she’ll take time to talk through your child’s specific challenges and help determine whether Foster Village DFW can provide sensory supports to make day-to-day life a little easier.
